The Big Lake Country Club Assisted Living home strives to provide the highest quality care to adults with emotional, behavioral and developmental problems in a positive working environment.
Responsibilities;
•Assesses the functional and psychological needs of the patient within the framework of his/her developmental stage, functional abilities, cultural milieu, and support network.
• Formulates a plan of intervention acceptable to the patient, family and medical team; proactively facilitates adjustment in the care plan or services when necessary to promote enhanced outcomes.
• Documents all interventions in the patient medial record both timely and accurately including all elements of the discharge plan.
• Provides advocacy, assistance, support, counseling and crisis intervention to patients and families. In the case of suspected abuse/neglect/exploitation, the Social Worker documents and reports where required. Assists with hospital-sponsored groups.
• Functions in a liaison role between the hospital and community in making community resources available to the patient and family.
• Collaborates with all members of the healthcare team to develop, manage and communicate discharge plans. Maintains current working knowledge of Medicare, Medicaid and other program benefits and criteria, particularly as they pertain to long-term and low-income patients.
• Provides professional education to staff.
